Our office accepts most major insurance plans as listed below. However, selfpay, co payments and deductibles are due at time of visit.

Attention Medicare Patient: Our physicians are not contracted with Medicare and payment is due at time of service. However, our office will gladly file your medical claims and reimbursement will be paid directly to you the patient.

Attention Medicaid Patient: Our physicians are not contracted with Medicaid, therefore payment is due at time of service. Filing is not done with Medicaid.

Workers’ Compensation and Company Billing: Once authorization for services has been received from the patient’s employer our billing office will be glad to file worker’s compensation, drug screens and company physical claims. New company accounts agreements can be set up by contacting our Company Account Specialist, Jeanette Brigman.

Motor Vehicle Injury Payment Policy: Most major health insurance companies DO NOT PAY the doctors for treating injuries you sustained in a motor vehicle accident. Payment must come from the automobile insurance company of the “at fault” driver. The determination of “at fault” may take up to a year or more and often involves litigation.

For the above reason, the following financial policy shall apply:
Payment is required at time of service. We accept cash, check, or major credit cards.
